This is your listing submission form from where users will be able to add their business listings on your website.
You can manage which fields appear here from Tevolution>> Custom Fields section in backend. You can change this message by editing the Submit Listing page in WordPress pages section.

One of the most important part for the business directory site is Submit listing page. Detailed information is available here. New user registration can be enabled from WordPress general settings -> ‘Anyone can register’ option. Enable the option
